Collaborative Care

Collaborating For The Best Patient Outcomes

Collaboration among primary care physicians and behavioral health providers has never been more essential for the delivery of integrated quality patient care. In 2019, there were an estimated 51.5 million adults aged 18 or older in the United States with any mental illness (AMI) within the past year (this number does not include substance use disorders).*

Practitioners serving members whose behavioral healthcare is managed by Magellan have access to expert behavioral health consultations by Magellan's master's-level care managers — all you need to do is call the number on the back your patient's insurance card. A care manager will work with you to determine the most appropriate resource based on patient need and preference, provider specialty, as well as other specific member requests or considerations. We encourage you to take advantage of such consultations provided as part of your patient's behavioral health benefits.

Also, as a resource to providers, we've developed a sample clinician coordination of care form (PDF).
